            Welcome to the Athletics program at Christian Brothers University.  The athletic trainer is responsible for determining if student athletes are medically cleared to participate.  The following seven tasks and forms must be completed and on file with the athletic trainer at the start of each school year for medical clearance to be granted to a particular player.  Items 1 and 2 are required as indicated and items 3-7 are required each academic year.  You are required to have at least $5000 in college sports injury coverage to play sports at CBU.  You are required to contact the athletics department immediately if during the year, there is a material change or update to the information that you provide on these forms.

 

Please print out, complete and mail in as much information as possible on these medical forms prior to your arrival on campus to expedite your medical clearance which allows you to start working out as soon as possible.

 

1. The completed pre-participation physical (first or fifth year applicable.)

2. Attend the orthopedic screening, which is a free visit with the team physician to discuss any ailments or problems regarding your participation in collegiate athletics. (First year at CBU applicable and traditionally the first days of fall sports and the first week of classes.  Contact your coach to learn date, time, and location.  Please hold this form until you have seen the school physician and had it signed.)

3. The completed Health insurance information/Authorization form

4. A photocopy (front and back) of the student-athlete's health insurance and prescription benefits cards (please submit all applicable cards if you are insured under more than one policy.)

5. The completed CBU waivers and releases form

6. The completed NCAA's required acknowledgement of insurance form, which must be signed by a parent if the student is a US citizen

7. The completed medical history form if this is your first year at CBU, OR the Follow-up Medical Questionnaire if this is your second through fifth season
